In short: PHR keeps your health records on your iPhone and in

your own private iCloud account. The developer cannot see, access, or collect your data.

There are no ads, no analytics, and no third-party tracking.

PHR is not medical advice; check anything important with your doctor.

Not medical advice - PHR helps you keep and view your own records. It does not diagnose or treat any condition. Always check with your doctor before making medical decisions.

What data the app handles

PHR is a personal health record keeping. Depending on what you choose to add, it stores:

- Records you enter yourself: vitals, lab results, conditions, medications, and vaccines.

- Documents you import: PDF files such as lab reports, and the text read from them.

- Apple Health data, if you connect it: steps, heart rate, body weight, blood pressure,

body temperature, oxygen saturation, and workouts. The app only reads this data; it does not write to Apple Health.

Where your data is stored

- On your device. All records are stored in the app's database on your iPhone.

- In your private iCloud account. Records sync through Apple's iCloud (CloudKit private database)

so they are backed up and available on your devices signed in to the same Apple Account. This storage belongs

to your iCloud account; the developer has no access to it.

- Vaccination QR codes (SMART Health Cards). When you import one, the card is decoded on your

iPhone. To check that the card is genuine, the app downloads the issuer's public signing keys from the

issuer's website (for example, your state registry or pharmacy). No health data or personal information is sent.

- Document text is read on your device. Text extraction from PDFs, including scanned pages,

is done on your iPhone using Apple's built-in frameworks. Documents are not uploaded to any server operated by the developer.

Ask PHR (the in-app assistant) runs on your device. Questions and answers are processed by

Apple's on-device model (Apple Intelligence). Your records and questions are not sent to the developer or to any AI service.

If you ask by voice, speech is converted to text on your iPhone.

App lock (Face ID or passcode)

You can turn on an optional lock for PHR in About → Privacy & Security.

- It uses Face ID or Touch ID (whichever your iPhone has), or your iPhone passcode. Your face and fingerprint data stay inside iOS: PHR is only told whether the check passed.

- PHR locks when you leave it, after the time you choose.

- Whether or not the lock is on, PHR hides your records in the app switcher and while your screen is being recorded or mirrored. iOS does not allow apps to block screenshots.

- While PHR is locked, Siri won’t read your results aloud.

- The lock is optional, and you can turn it off at any time.

Encrypted backup

You can save all your records, documents and photos into one backup file, locked with a password you choose (at least 10 characters).

- The file is encrypted on your iPhone before it is saved. Only your password can open it.

- A lost password cannot be recovered. Without it, nobody can open the backup, including the developer.

- You decide where it goes: Files, AirDrop, your computer or another place. PHR does not upload it anywhere, and the developer never receives it.

- Restoring adds records that are missing and never changes or removes records that are already on your iPhone.

- Copies of the backup file that you save elsewhere are yours to keep or delete.

Subscription (PHR Premium)

PHR is free to download. After a free trial, using the app needs a subscription, sold through the App Store.

- Plans: a monthly plan that starts with a 7-day free trial and renews automatically until you cancel, and a one-year pass that is paid once and does not renew. Prices and terms are shown in the app before you buy.

- Apple handles payment. Your payment details and purchase history are held by Apple under your Apple Account. The developer never sees them. You can manage or cancel in Settings → your name → Subscriptions.

What PHR learns: only whether your plan is active and when it ends. Your iPhone gets this from the App Store, and PHR does not send it anywhere or link it to your records.

- Your records stay yours. If a plan ends, PHR does not delete anything. Your records stay on your iPhone, and you can always make an encrypted backup of them, even without a subscription.

- Apple's own terms and privacy policy apply to purchases.

What the developer collects

Nothing. The developer does not operate any server that receives your data, and the app contains no

analytics, advertising, or third-party SDKs. Your health data is never sold, shared, or used for advertising,

marketing, or data mining.

If you choose to share diagnostics with app developers in your iPhone settings, Apple may provide

anonymized crash reports to the developer. These do not contain your health records.

Apple Health (HealthKit)

Access to Apple Health is optional and only happens if you tap Connect Apple Health and grant permission.

You can change or revoke this at any time in Settings → Health → Data Access & Devices → PHR.

Data read from Apple Health is used only to show your own records and trends in the app.

Deleting your data

- Individual records and documents: delete them in the app. Deletions also sync to your iCloud.

- All app data on a device: delete the app from your iPhone.

- The iCloud copy: go to Settings → [your name] → iCloud → Manage Account Storage,

select PHR, and delete its data.

Because the developer never receives your data, there is nothing held by the developer to request deletion of.

Not medical advice - PHR helps you keep and view your own records. It does not diagnose or treat any condition. Always check with your doctor before making medical decisions.

Children

PHR is not directed to children under 13 and does not knowingly collect any data from them.

Changes to this policy

If the way the app handles data changes, this page will be updated and the effective date above will change.
